Tensioner Pulleys Backing Out- Thoughts?

Tensioner Pulleys Backing Out- Thoughts?
So I am having a hell of a time with my tensioner pulleys .. both the PS, Alternator side & the AC condenser side.
If I don't put them up especially tight to the belts they back down ... this has happened twice now - the pulley on the tensioner will rattle itself down to where it will not turn the belt, which has now left me stranded because it wasn't making the alternator go enough to recharge the battery.
When I tried to really torque it up, it kept contact, but then it backed off the nut and I lost the pulley off the tension bolt/mechanism thing itself.
Has anyone ran into either of these problems, either the pulley rattling back down the threads or almost losing the pulley off the front of the mechanism?
Any thoughts on a solution?

Are you tightening the 'lock' nut in the center of the pulley?
If you think your pulley assembly has crapped out on you, I have one I'll sell you super inexpensive from a block that I parted out.

Yes I am doing both the tension from the bottom & the nut in the center .. still having issues... wondering if i might try to put a nut behind each to back up to?
Is it the condenser side or power steering/alternator side?
