I would suggest using the middle leg to do the shifting when both hands are occupied. You don't want to risk crashing if you let go of steering wheel.

OP if your focused on shifting while your "working with your hands", your doing it wrong.

Also shifting with your left hand is dangerous and immature, if she really wants the D, pull over in a parking lot (preferably away from lights or under them if ur into that) and beat those cheeks on the hood or the trunk.
