Self adjusting ECU - myth or true?

To stop more arguing, yes, the ECU is a self learn ecu, it will adjust on its own.
also, there have been reports of ecu's that have been reflashed, that when they are reset for a check engine light or similar issue, they lose the re-program or the reflash, and that they end up basically back at the stock ecu settings. not sure who reflashed these ecu's or how, just relaying what some customers have said.
The ecu from my understanding , constantly adjusts itself.

so the ecu is self-learning, continuously, over-and-over, forever until it dies. great! so why do people run dangerously lean after extensive bolt-ons (ie headers, cats, plenum, intake, etc.). the worst was in the upper 14's, lower 15's. 16 and the car goes boom. shouldn't the ecu richen the dangerously lean mixture? could it be that the it's outside of the ecu's parameters, and it jus' can't adjust it that much? but even then, it should be able to richen it out jus' a little bit, right?
as for the reflash, i don't think the ecu will learn it's way "out of it" since a reflash is actually going into the code, and reprogramming it. when the ecu learns, it's going off of the new program into it.
