Default axel help

Ok this is the problem my axel nut on the passenger rear is coming loose this is the
3rd time the nut has come loose. i tighted it twice this last time it trashed the wheel bearing so to the dealer it went, i had to replace the wheel bearing and had the tsb done for the axel why it was out that was last week. Today i noticed it was showing signs of the nut coming loose again . I dont know what to do with it any ideas.

The nut needs to be tightened to 152 - 202 ft of torque and also there should be a Cotter pin to stop the nut from coming loose

the dealer tightend it down i dunno
