Default powerlab 3/8" plenum spacer help!!!!!!

I bought a used powerlab spacer from a friend and I dont think it has all the bolts... Can anyone help me with the amount of bolts, size, lenght, or if you have the instructions that have what is included in the kit.

Since this is a copycat product, you can get the bolt package from Motordyne and it will work.
