A quick warning........
If you hold the button for "I'm not really sure, probably between 15-20 seconds in 5th on the highway"
You will end up with this, as well as other minor things like a blown HG and a partailly messed up cylinder wall.

Every other precaution was taken, running a 100shot w/window switch, wot switch, and even a fuel pressure cut-off (not that that will always prevent anything, but it is slightly good measure)

the problem was you were spraying in 5th. I never spray past 4th unless in at the drag strip then its just a lil bit in 5th. The problem w/ spraying in 5th is its just too long and putting all the pressure on the motor for that long ALWAYS=bang.

Did this customer of yours just fix/replace everything with stock parts or did he upgrade? If upgraded, what parts did he use and why? Adding in prices and down time would also be nice.
Thats a shame...Personally I don't see anything wrong with using N2o under the conditions that you have respect for the product and know the limits of your vehicle.
With that being said, taking full safety precautions is a must to include knowing when it is safe to use it.
So, I don't feel bad for the guy because it is a good lesson learned.

Couldn't of said it better...even though my setup was for drag racing I was doing fine in the 1/4 mile until I shifted to 4th (5AT) and took it to 7k and boom.We were trying to get into the 10's with the first stock internals/5AT. But thats how we roll at PF. I can't wait till the built shortblock is done but no more 75 shot just a gentle 35 shot from now on.

I recall reading that he only sprayed it in his last gear, but I could be misremembering things.
