This was pulled off a google search, but i've seen rotors with sections completely broken off.
Few years back I had a slotted rotor on my WRX that cracked exactly like that pic. I don't remember who made them but I know they were a name brand. I think stoptec, not sure tho it's been a while(8ish years).

I bought a pair of new "brembo" rotors off of ebay from my old Eagle Talon. Car shook to all hell after I put them on. Took it to my mechanic and he said I had some cheap brembo knock offs from china. They were warped and had to be turned multiple times to straighten them.
Stay away from ebay rotor venders.
