It may very well be under the Engine cover ---I dont recall off hand and my plenum is powder coated with no cover.....AND.....I use that port for an in car Boost/Vacuum gage
Maybe, but generally not unless the rubber cap is brittle and cracked from heat. A upper plenum/spacer leak is generally around the perimeter or lower plenum Gasket
Not if the plug is gone...could have easily come off during install and been forgotten...

Check and make sure the gasket isn't folded on itself anywhere, check and make sure nothing, such as the rubber oil filler cone, is in the way of the plenum, and then make sure all your bolts are torqued down to spec.
