Default Burnt Clutch?

So I'm starting to get reasons to believe my oem clutch is starting to wear out. The problem I'm experiencing is a loss of power in the higher gears (3rd to 6th).

I can be cruising on the highway at around 50mph and when I try to "punch it" and give it more gas like when I'm trying to merge into a lane, my rpms would jump like they would normally but then die down a second later as if power wasn't transmitting. I suspect either a worn out clutch (at 86k miles, I don't doubt it) or fuel/air problems.

I have minimal mods done: Injen CAI, HKS exhaust and Berk HFCs.

Below is a vid of it happening. I think I was cruising in 4th. I'm holding the throttle constant before giving it more gas.

Edit: Also my CEL did come on before and the code checked out to be a running lean code. I drove it some more and resetted the ECU a couple times thinking AFR would remap and the CEL went away for good.

The clutch is slipping. It is not holding the flywheel. Crankshaft and flywheel goes round-and-round - drive wheels do not.

Okey doke, that's all the confirmation I need.

Figured it was the clutch (previous owner was a woman) but wanted a 3rd opinion. Thanks!
