Default Intermittent fault & running AFR running lean

Hi Guys,

Tried to do some research on this with no avail.
Car is vortech supercharged and at times recently it has started developing a fault where the revs will not rise above 5krpm and the AFR gauge starts to rise up and then null causing the rev limit to cut there. I've attached 2 gifs to this thread. One with the car running well (most of the time). One with the car showing the fault.

Things we have tested so far:
- Possible fuel pressure issue. New pump, Aeromotive 340 was installed a few weeks ago. The fault did develop after then but i've had it removed and rechecked and it doesn't appear to be related to the install.
- Venturi swirl jet has been drilled out to 2.5mm - fault has not happened since then.
- There are no codes on the ECM/BCM which are showing which could rule out a sensor issue.

The car most of the time seems to run perfectly normally. It was checked on the dyno yesterday and ran fine there too. The tuner said it did have an irregular fuel pressure on idle and thinks it could potentially be the fuel damper.

The trouble is it's intermittent. The fault has not occurred all week. I have an uprev licence, I've been data logging and trying to find where the fault could be to no avail. Anyone have any thoughts on how to pin point issue.
