Default Sudden grinding of late

My car has over four thousand miles on it, it is a manual touring. In the last few hundred miles I've grinded the gears (at least I think that's what happened) shifting into second at around 6,000 rpm. I'm not sure what I'm doing wrong, I am a beginner driver but I learned pretty well and I'm better now than I was in the first few thousand miles. The first two times instead of going into second I just heard a loud CRACK!!! and when I let go it would be in neutral. On the last time there was an actual grinding noise and then it went into second. Is there something I am doing wrong? Could it be my car, perhaps a sign of clutch burn-out?
