Check the engine oxygen sensor. since it's occuring after the car warms up...i'm thinking that there is a problem switching from open loop to closed loop
when the car is warming up...it's running on open loop off mapped data.
when it warms up and switches to closed loop, the oxygen sensor is giving bad voltages or some kind of bad readings.
just my guess.

Also check the MAF. I had the same problem (low/rough idle when warm) and was able to get a new MAF (through warranty, luckily!) Usually, the MAF just requires cleaning. Apparently, my MAF was actually bad (which seems to be somewhat common here).

For a third possibility, after installing my plenum spacer I was having the exact same problem, and it appeared to be caused by vacuum leak from the plenum bolts not being tightened all the way.
