Default Weird rotating rubbing sound from rear

Hey guys, so I searched for this issue, but its hard to narrow down and I'm not sure that I found another thread thats the exact same issue.

I have this weird scraping rubbing sound coming from the rear of the car, and its much louder at low speeds, less than 40mph, and gets even louder when turning at speed. Additionally, its a rotating sound because it gets quicker with the rotation of the wheels, so by the time I'm past 40mph you can't really hear it because its rotating so fast.

I'm not sure what the heck it is. I took off my rear wheels, took apart my brakes, lubed up the pins, put the wheels back on and its still there. The tires are winter tires, 245/45/18 on factory wheels with 10mm spacers that I had on there from my summer wheels. The spacers are Ichiba Type 1, hubcentric with extended hub screws. Stock suspension.

What could the problem be?

I replaced my pads 2000 miles ago with NEW Nismo pads all around. The rotors are factory around 46000 miles.

What is your lbs-ft of torque on the lugnuts?

Also what is it on the extended screws?

It does not pull to one side.

Also when i'm coming to a complete stop it makes a very loud rubbing noise when its coming to a 5mph-stop. It makes the whole car vibrate under 5mph. This is what leads me to believe its a warped rotor.

Another thing to mention, I took out my rear oem pads a couple thousand miles ago and I found this hole in the pad. Where could something like this come from? Any ideas so I know when I take my rear brakes apart to install the new rotors?

I already bought new rotors and ceramic pads that I am going to be installing on Monday for good measure. I'm going to take apart the entire brake system, maybe get some stainless lines too and bleed all the brakes.

Thanks for your help guys.

I'm not sure what the lbs ft is on the lugnuts. By extended screws do you mean the caliper bolt/pins? I'll probably go pick up a torque wrench for the install. What lbs-ft do you recommend for the caliper bolts and lug nuts?

Update:

so this has been hell week! Turns out I had a bad wheel bearing driver side rear. I ended up changing all my rotors and pads for no reason. Right now I have my axle out and whole hub assembly out. I'm having the new bearing being pressed in right now, and I'll be re-assembling the rear tonight. Lets hope all is well! Thanks for the help guys.
