Default Need help with identifying if Cluth/Transmission issues?

Need a little push in the right direction as to what the problem could be before i bite the bullet and just take the car to nissan for diagnostic...

Symptom:
I shift into gear(any), accelerate as normal(daily driving), i let off the accel pedal because of traffic conditions and/or just because i want to slowdown, then i press the accel pedal and the car/tranny jerks as it begins to accelerate again. i didnt touch the clutch pedal in any way, this is simply letting off the gas and then pushing it again....

thats the best i can describe it...what do you guys think?

At what speeds is this happening? Does it happen in highway speeds or when your doing like 10mph?

it happens in general, any gear, any speed...though i have to say its more noticeable at lower speeds(1-3rd gear)
