Default Idling and Rev issue

So I've done a bit of thinking, and I can't figure out why my car is doing this.

Here are the symptoms:

When idling with or without heating/AC the rev's will periodically drop quite a bit, like its choking almost, and sometimes will die

When holding the gas pedal at a specific point without load (ie not in gear) it will randomly drop 250rpm or more in the same choking fashion
-- Additional note: This is only if I hold it below 2000rpm, above 2k it does not have this issue

When holding the gas pedal at a specific point with load it will cause hard jerking, and of course act like its choking.

When doing a cold start it sounds like its misfiring when I rev it, goes away after a minute. I don't mean revving it all to hell, I mean going up to 2500rpm

Stuff I've done:
Changed Oil - Castrol 5w30 synthetic
Changed Diff fluid - Mobil 1 75w90 synthetic (helped a bit with the jerking obviously)
Checked Air filter - its clean
Always used 91 octane gas

It throws no codes, and I have no idea what the issue might be. My next thing I was thinking of doing was changing spark plugs.

Any ideas guys?

MAF either not connected properly or has gone bad.

Haven't checked that, but wouldn't it throw a code?

Not necessarily. If it's just starting to go bad, it might not throw a code. If it is going bad, it will get worse.
I had a car with a bad MAF and when I would try to accelerate, the engine would "stall", then take off like a bat out of you-know-where.

Only at low RPMs do I have an issue, never above 2000

Just as an update, I reset the ECU, and so on.

I found that this only happens when I have my heater/AC on.

Surprisingly my voltage dosen't drop below 14, should I upgrade my grounds and hot wire to the alternator?
