Why does my throwout bearing keep going out?

LOL, just opened up the clutch today. there were chunks of the ceramic material missing from a few of the pucks. hopefully im using the right terminology. the chucks were chard onto the flywheel which was then cutting deep grooves into the other pucks. it looked like total crap. the grinding noise was coming from where the worn down puck, basically down to the metal portions, were contacting the flywheel. i'll pictures when i get a chance and post them up.

^^ when i took my clutch apart to replace the t/o bearing for the first time my clutch was fine, no hot spots, no unusual wear, clutch itself had plenty of life left nothing like you posted. My throwout bearing was broken/shattered and the little ball bearings were all over the place tho. It seems like it seized or gave out.
any update on this? Anyone else have this problem? Mine feels like it isn't releasing the clutch sometimes. I still have this problem.
