Default Idle pulley flew off while driving

I was driving around and my idle pulley for the alternator , powersteering belt flew off.

So what would cause this, nut was torqued down.

What one, there are 2 on that belt. The lower one on the tensioner or upper useless one that you can just delete.

Threads look fine and pulley is in tact bearings move freely. I ordered the missing parts and I'll really tighten it down this time and see what happens
