Cut was $5. Program was $55, key was $39 or something (forgot) on Ebay. I had to have all my current keys present during the time of programming. I guess they do them all at once or something. I have a slightly different version than the one the OP posted. I have this version:

Read my thread. By the way, don't buy that flipkey, it's a rip off, I got mine for $19.99 USD. I made this about a week and a half ago. It works great, very sturdy and no problems so far. The whole swap takes about 20 min and a trip to the dealership to get the keys cut.

Im now trying to decide which one is better quality....
The one from Honk Kong (square one) Or the one from the guy on G35 Driver (rounded one)
The square one is the only one i have found for $20 shipped...thats a good price. Anybody know where to pick up the rounded for the same price?

The square one requires you to cut your current key and move the transponder to inside the square chassis. Either one should be ok; I got the one with the transponder so I can have an additional key to my set incase I should ever lose one.

I think the reason why the rounded one (the one I have) is a little more expensive is because it comes with the transponder already built in and you'll need to get it reprogrammed.

The round one seems to be of better quality than the square...but thats just based on looks in the pics.
