Default sound of hitting rev limiter

i'm hearing the sound of hitting rev limiter around 4500 RPM.
This doesn't happen all the time though. when i launch straight away, shift to 2nd and drop the clutch, i hear the sound.
With the sound of wa dah dah dah dah!!, the car loses power.
OH! is this caused by slipping clutch? i tested it out on the freeway (going around 60mph, and downshift to 3rd?. no slipping there).

Pull codes from the ecu, you may have jumped a tooth on the flywheel.

So the issue is only from first to second? Is it at hard shift or in second as it gains speed? Happens every time or only once in a while?

only from first to second. it happens when I hard shift n drop the clutch.
it happens most of the time in straightaway. I have to launch hard in first gear, then hard shift. happens most of the time.
