I guess I always expected the OEM studs and/or the lug nuts that come with the spacers to stuck out beyond the spacers, which is why I thought I would always have to make sure my wheels had those cutouts between the lug holes if I wanted to run spacers. But grinding down the OEM lugs would work too.

It's not "old style" vs. new style. The one with the 4pt key is used for the smaller spacers (i.e. 10mm) because of the thickenss. The ones with a bolt are for larger spacers (i.e.20mm+). KICS is the same way, but I haven't heard any complaints about the 4pt bolt used on KICS spacers. No idea about the quality of the KICS 4pt bolt.
